GAPP Program

Professional caregiver providing compassionate home health care to senior patient

GAPP Program (Georgia Pediatric Program)

The Georgia Pediatric Program (GAPP) provides Georgia Medicaid-Funded medically necessary in-home care for children and young adults (up to age 21) with complex medical needs, allowing them to receive high-quality care in the comfort and safety of their own homes.

Our GAPP services are delivered by licensed nurses and trained caregivers who specialize in pediatric care. We work closely with physicians, care coordinators, and families to create individualized care plans that support each child’s medical needs while promoting stability, routine, and quality of life.

By providing skilled, one-on-one care at home, the GAPP program helps reduce hospitalizations and allows families to focus on what matters most — being a family.

GAPP services may include:

  • Skilled nursing care (RN/LPN)
  • Medication administration and monitoring
  • Tracheostomy and ventilator care
  • Tube feeding management
  • Seizure monitoring and support
  • Assistance with activities of daily living
  • Ongoing communication with physicians and care teams

We understand that caring for a medically fragile child can be overwhelming. Our team is committed to providing dependable, compassionate care while offering families peace of mind and consistent support every step of the way.
